Under the general supervision of the Regional Director the Community Nurse is responsible for managing the medical needs of people being supported by the Charles Lea Center.
**Please note: We will consider Part-time and/or Independent Contractor for this position**

Follow up on compliance & medical recommendations timely to assure people?s needs are being met.
Ensure people supported attend medical appointments and all follow up appointments.
Attend medical appointments as needed, especially appointments for surgeries or major illnesses.
Complete assessment (annual, monthly or as needed) for people assigned. (Hair, nails, teeth, toileting, diet, effectiveness of medication, meals, skin, wound care, pressure sores, new drug, illnesses, weight, injuries and any other deemed necessary). Recommend action and/or training and follow-up.
Complete health care plans as needed for individuals supported. Complete outlined steps of health care plan or provide necessary training and ongoing supervision/oversight to direct support staff completing aspects of the health care plan.
Evaluate blood pressure and blood sugar results for people whom this testing is required.
Attend team meetings and formulate call team meeting when medical issues arise to seek resolutions and recommendations on medical or health related concerns.
Ensure testing reports have been communicated to ordering health care provider and recommendations have been followed for any order change.
Check adaptive equipment monthly to make sure all devices are in good working condition, clean and staff understands how it?s used and cleaned (feeding devices, lifts, wheelchairs, communication devices, shower chairs, special shoes, spoons, plates, cups, clothing etc.)
Monitor meals and diets in each home as needed. Check diets, special diets, menu, food availability, food prep equipment, swallowing, calorie intake and fluid intake. Provide training to staff and people supported if concerns arise and document training.
Assist in training of Community Support staff in proper medication administration procedures. Monitor at least one medication pass per home quarterly. Provide training to staff if concerns arise and document training.
Administer medications to assigned programs if the need arises. Follow policy and procedure for correct administration and documentation.
Follow-up to ensure pharmacy orders have been properly documented on the MAR for the programs.
Ensure new medications are available to people within 24 hours of receiving the order.
Provide on-call support to programs and advise staff on appropriate treatment and care. May need to make an on-site visit as needed to determine if person needs emergency medical care or to complete follow-up.
Other duties as assigned.
Current LPN/RN License, in good standing with the State Licensing Board
Valid Driver’s License
Good people skill especially with people supported, families, clinical staff and co-workers
Good written and verbal communication skills
Knowledge of medical terminology
Good computer skills
Ability to exert physical effort, standing, lifting, and carrying materials or equipment
